Report Yellow Cards: Please record any and all cards players or coaches receive
Player Pass: Work with your AGC. Bring 2 copies of any roster that you are borrowing from. If 5/6 is borrowing from 3/4, make sure there are photos on 3/4. AGCs or Coaches - FILL THIS FORM out - and CYSA will send along to MYSL. This can be done BEFORE or AFTER the game. Not required, but it is helpful to email the other coach prior to the game.
