Report Yellow Cards: Please record any and all cards players or coaches receive
Player Pass: Work with your AGC. Bring 2 copies of any roster that you are borrowing from. If 5/6 is borrowing from 3/4, make sure there are photos on 3/4. AGCs or Coaches - FILL THIS FORM out - and CYSA will send along to MYSL. This can be done BEFORE or AFTER the game. Not required, but it is helpful to email the other coach prior to the game. Who can you borrow? Same age group, same division or lower. Down an age group, you can go up a division. Example: Boys78 D4, can borrow from B78 D4,5 and B56 D3,4,5.
